Having an image performed at a CDI facility allows 100% coverage for (most) medical imaging exams such as MRI’s, CT’s and X-Rays.

There are three different ways you can schedule an appointment at a CDI facility:
1. Call MAP (Medical Advocate Program) at Local 49’s dedicated number 866-573-5745, or call CDI’s Labor Appointment Line at (866) 765-7138.
2. Your doctor sends a written prescription for the exam. Please work with them first to ensure CDI has all the information required to guide your care.
3. Pre-register for your exam on CDI’s Patient Portal – a secure login is required. Request access when CDI sets up your appointment. www.mycdiexam.com
